Medical history form

To provide you with the best possible support, I will take time for you and your story. The medical history form will help me better understand your current needs, health issues, and personal goals. Please take a few minutes to complete it beforehand in peace and quiet – confidentially and at your own pace. This way, I can address your individual needs and optimally structure your session. You can send the form to me in advance by email or bring it with you at the start of the retreat.
